California Health Collaborative offers two types of programs, diabetes prevention (DPP) and management classes (DEEP) throughout Fresno County.
DEEP educates and empowers people with diabetes through a six-week diabetes management class series. These classes focus on the following:
■ Diabetes risk: factors
■ Complications
■ Nutrition
■ Physical activity
■ Use of glucose meter and medications
■ Building partnership with a diabetes healthcare team
■ Psychological effects of illness
Classes are available in both English and Spanish. No doctor referral is required.
The diabetes self-management classes are held in health centers, schools, community centers, apartments, etc. Currently, classes are available online. To learn about the upcoming diabetes self-management classes, please contact Gina Falcon at (559) 244-4544.
Diabetes Prevention Program
Partnering with Solera and Saint Agnes Medical Center, the California Health Collaborative hosts the Diabetes Prevention Program (DPP) to prevent type 2 diabetes in at-risk individuals. To participate in this 12-month program, the following is required:
- Be at least 18 years old and
- Have no previous diagnosis of type 1 or type 2 diabetes
